6/9/2023 EnchantaVaughn Approved Behavior: During todayâs visit, Ben-Nita appeared to be in a shifted mood, as evidenced by her irritable emotions and puzzled facial expressions. Ben-Nita described a pattern of consistent misfortunate events that happened today. Ben-Nita expresses feelings of bad luck due to certain things not going her way. Ben-Nita verbalizes different complaints but utilizes her coping skills properly once she was in a more stable mindset. Ben-Nita was supported for demonstrating a decrease in level of paranoia. Behavior: Ben-Nita demonstrates ability to hold herself accountable and adapt to new challenges. Ben-Nita was reminded to utilize relaxation techniques and breathe so that she could stay calm and focus in the session. Ben-Nita expressed needing more help and agreed to be less emotional and more logical with handling stressful situations at home during this time. Ben-Nita was appreciative of the therapeutic services and having someone to depend on that understood her on serious and personal level. Intervention: QMHPC addressed Ben-Nitaâs negative behavior right away. QMHPC allowed Ben-Nita to individually process what occurred throughout the day. QMHPC validated Ben-Nitaâs feelings but explained the importance of utilizing her coping skills. QMHPC stated âYou have to learn to ignore others and not be so easily triggered.â QMHPC isolated Ben-Nita from distractions to decrease disruptive behaviors and to reduce excessive talking. QMHPC explored Ben-Nitaâs capability to cope with her stressors on her own. Intervention: QMHPC verbally identified Ben-Nita's impulsiveness and encouraged Ben-Nita to use self-control strategies. QMHPC brainstormed strategies with Ben-Nita and encourage her to implement them on a daily basis. QMHPC also provided mediation and relaxation techniques to maintain appropriate behaviors. QMHPC explained the importance of being on task and refraining from outside distractions. QMHPC reviewed and gave feedback regarding ADHD and its causes and affect toward Ben-Nita well-being and academic functioning. Response: Ben-Nita actively participated with QMHPC as they discussed and processed her feelings. Ben-Nita communicated that it is often hard to remain positive at all times about herself when she doesnât always have the best things or necessities that she needs. Ben-Nita discussed numerous situations where she had to adapt even when she didnât want to but was thankful for the changes. Ben-Nita was receptive as QMHPC demonstrated better ways to function in the home setting. Response: Ben-Nita displayed active behaviors in the second half of the session. Ben-Nita engaged in self-reflection, discussing good and bad parts of her day. Ben-Nita chose to get herself together and participate. Ben-Nita communicated that she feels that she will be more successful, due to having a woman as a counselor. Mom expressed to the counselor that though Ben-Nitaâs behavior has improved she still has to remind Ben-Nita about remaining consistent with her reactions and perceptions.
